For repair of faulty equipment, please print a copy of the Return Authorization Request form (see pdf.) and fill in the following information: P.O. number, Bill To, Ship To, the return shipping method, quantity, model number and description of the problem with each unit. Fax the request to 800-697-6260 and we will provide a return authorization number via fax to you (please circle return fax number and provide contact name).

Please enclose a copy of the completed Return Authorization form with your equipment. Allow for 7-10 business days for completion of repair(s).
